The information collected and displayed on this website is used to update your records in the Attentive Health program in which you are enrolled (i.e. the “Passport to Health” wellness incentive program sponsored by your employer) and to respond to you in the event that you contact us. We will not share your information with any third party outside of our organization, other than what is necessary to fulfill your request, e.g. to confirm your eligibility for discounted merchandise. We will not sell or rent this information to anyone.

 

Unless you ask us not to, we may contact you via email in the future to inform you about the Attentive Health program in which you are enrolled, or changes to this Privacy Policy.

• Participation, Incentives and Rewards: Attentive Health may disclose your Personal Information to your health plan, which may be administered by your employer, in order for your employer or health plan to provide you or your spouse/same-sex domestic partner with incentives and rewards for participation in Attentive Health programs. We will limit the amount of Personal Information we share to the minimum necessary for you to receive the incentives and rewards.

• Agents, Contractors, and Other Service Providers: In order to provide you with the services on the Site, we may disclose your Personal Information to our agents, contractors, or other service providers who perform services on our behalf, such as incentive fulfilment, web hosting, translation or data storage. These third parties may also collect Personal Information on our behalf. Attentive Health will ensure that any agent, contractor, or other service provider with whom we share Personal Information agrees to safeguard it in substantially the same manner as Attentive Health has described in this Privacy Policy, but in any event in accordance with all applicable laws and regulations.

We may not limit the third parties' use of the Aggregate Information, except that we do require third parties to whom we disclose Aggregate Information to agree that they will not attempt to make this information personally identifiable by combining it with other databases.
